Our equine activities often require the completion of a variety of forms. We have put them all in one place on this page for your convenience:

Required Forms for Participation in MHA Programs

Download an MHA membership form here. (Help support the activities of MHA! Required if you plan on participating in MHA activities.)

MHA Agreement and Liability Release Form (to engage in one or more equine activities sponsored by MHA, including riding lessons, clinics, boarding, horse back riding and the rental and use of tack and other equine equipment)

City of Mackinac Island Release, Assumption of Risk & Indemnification Agreement (to participate in the horseback riding program, in the use of the Great Turtle Park Riding Ring, and
other activities connected with this program)

When leasing a horse, you must fill out the following documents:

• Mackinac Horsemen’s Association Equine Lease Deposit form. This form and a check made out to Mackinac Horsemen’s Association in the amount of $200.00 must be returned no later than April 15th.
• MHA Agreement and Liability Release Form
• Mackinac Horsemen’s Association Boarding Contract and Liability Release
• Mackinac Horsemen’s Association Equine Lease
• City of Mackinac Island Release, Assumption of Risk and Indemnification Agreement
• Mackinac Horsemen’s Association Agreement and Liability Waiver and Release Form
• MHA Membership Form
• If under 18 years of age:
• 4-H Membership Form, Mackinac County, 2008
• MHA & 4H Horse Club Handbook. Please read and sign with
  your child.
